Just pick up a BSV Cash Plus Application at your local branch. Fill
it out, sign it, then bring it to your nearest BSV branch, or fax it
to (434) 246-2077, or mail it to us at: BSV Bankcard Center, P. O. Box 40, Carson, VA 23830.
Your card will arrive within 10 days or so, and the PIN will arrive
shortly thereafter.

terminals in supermarkets, gas stations and other merchant
